Direct billing means the clinic submits an eligible claim to your insurer on your behalf. It can reduce the amount of paperwork you need to do after an appointment, but it does not change the rules of your benefits plan.
What the insurer still decides
Your insurer decides whether the service is eligible, whether a referral is required, how much is reimbursed and whether a deductible, co-insurance amount or remaining balance applies.
Why coverage can vary between patients
Two people with the same insurance company can have very different plans. Employer group contracts, annual maximums and per-visit limits can all differ.
For that reason, direct billing should never be presented as a guarantee that an appointment will be fully paid by insurance.
